Appointments

Office Hours: Monday to Friday 8:30 am – 4:30 pm (weekly schedule can vary).

This is a request for appointment only.  Our staff will confirm all appointments. We ask that you arrive 15 minutes before the appointment time to complete a Health History form if provided in advance and your dental insurance information (benefit booklet if available) with you to your first visit in our office.

In addition, please bring a list of all medication (if any) you are currently taking.
